Free Patient Admission & Discharge Register – Google Sheets

A patient registration and discharge system for small hospitals, nursing homes and clinics, running entirely on Google Sheets. Admissions are recorded department by department, edited as the stay progresses, and pulled back out as dated reports.

Registration

The intake form captures IP number, patient name, sex, unit, KSP ID, case number, registration number, phone and date of admission, along with the amount. The department dropdown is not hard-coded: it is built from the sheets in your spreadsheet, so adding a third department means adding a sheet named for it and refreshing the app.

The dates that matter

Enhancement and discharge dates are usually unknown at admission, so they are left to the edit screen. The app refuses a discharge or enhancement date earlier than the admission date — a small check that stops the kind of typo which quietly breaks every length-of-stay report afterwards.

Finding and reporting

Search a serial number, mobile number or IP number to pull one record, or leave the box empty to list the whole department. The reports screen runs across all departments or one, filtered by date range, and says plainly when nothing matches instead of showing an empty grid. Any result can be copied, downloaded as Excel or PDF, or printed for the file.
