Free Patient Registration Form with Auto ID – Google Sheets

R39 is a patient registration form that a clinic can put online and leave running. Someone fills it in from their own phone, it lands in your sheet with a patient ID already assigned, and they get an email back confirming exactly what they submitted.

Validation where it matters

The date of birth is entered as separate values and checked properly: a month of 13 is refused, a year of 1899 is refused. Those two rules catch the majority of the nonsense dates that otherwise end up in a patient record and are painful to correct later. Required fields are checked before anything submits, so a half-filled registration never reaches the sheet.

Built for blood work as well as intake

Alongside the usual details, the form captures blood group, whether the person is a donor or a receiver, and their sugar level as low, normal or high. That makes it usable as a blood donor register, not only a patient intake form. The concern dropdown — billing, enquiry, product, technical — is a list you extend to match how your clinic actually sorts arrivals.

The confirmation email

On submission the patient is emailed everything they entered. It gives them a record, it gives them a chance to spot a mistyped phone number, and it means nobody has to ring the desk to ask whether the form went through.

Patient IDs you control

Each record gets a unique ID that increments automatically. Both the starting number and the step are adjustable, so you can begin at whatever number continues your existing records rather than restarting at one.

Setting it up

Copy the sheet, open Apps Script, deploy as a web app and grant the permission it needs to send the confirmation emails.
