Free Appointment Booking Form – Google Sheets Apps Script

An appointment booking form that writes bookings into a Google Sheet. Someone fills the form on the web, and the request appears as a row you can sort, filter and work from. No booking platform, no database server, no monthly fee — the spreadsheet you already open every day is the booking list.

What it does

The form is built with HTML, CSS and Bootstrap, so it is responsive and looks right on a phone, which is where most booking requests actually come from. On submit, the data is written to Google Sheets through Apps Script and the visitor gets a clean confirmation popup rather than a browser alert — a small thing that decides whether a form feels finished or unfinished. Because every booking is a row, you can build whatever you need on top: a filter by date, a pivot by service, or a second sheet that flags double-bookings.
