Free Filter Table with Add Entry Form – Google Apps Script

R30 is the filtered table with a way to add to it. The viewer version in this catalogue shows and filters what is in your sheet; this one puts an Add Entry button next to it, so the same screen that answers questions can also record new ones.

The form comes from your columns

Add Entry opens a form built from the sheet's columns — no field list anywhere in the code. Insert a new column in the spreadsheet and it appears twice on the next refresh: as a field on the form and as a filter above the table. That is the trick worth understanding, because it means the app never drifts out of step with the sheet.

Status as a colour, not a word

Values in the status column render as coloured badges — paid one colour, unpaid another, declined another — so scanning a long list is done by eye rather than by reading. The colours are set by a condition in the code, and adding another status is adding another branch, which the walkthrough shows.

The filtering underneath

Every column still gets its own dropdown, built from the values present with duplicates collapsed, and the filters combine. A combination with nothing behind it says data not found rather than showing an empty grid, and Reset clears them all at once. The demo runs against roughly a thousand rows, narrowing to four hundred on a single filter without a pause.

Setting it up

Copy the sheet, the code comes with it, deploy as a web app and grant the permission. A submitted record confirms on screen and appears in the table once the data reloads.
