Free Webcam Photo Capture Form – Google Sheets & Drive

A web form that takes a photo as part of the submission. The picture is captured from the camera, uploaded to Google Drive, and the form data goes to Google Sheets with the image linked to its row — so the record and the photo never get separated.

What it does

The form runs in the browser and opens the webcam or phone camera. The user takes the picture, sees a live preview before committing to it, and submits. Apps Script writes the row into Google Sheets and pushes the image into Google Drive, storing the link alongside the data. That link is the whole value: a photo that arrives separately, by email or message, has to be matched up by hand later, and often never is.
